If you are caring for an elderly person with multiple needs, you do not need to go to multiple locations. Being an adult caregiver is hard work. Monitoring prescription drugs, coordinating appointments, providing transportation -- the tasks never seem to end. Fortunately, there is one source where you can access all the services you need to care for loved ones with complex medical needs -- the Center for Healthy Aging.
The Center for Healthy Aging is a multidisciplinary, outpatient medical clinic, providing specialty consultations to geriatric patients. The clinical team at the Center works in collaboration with primary care physicians and nurse practitioners in order to help manage some of the most challenging patients, often with multiple or chronic health issues related to aging.
The Center is focused on meeting the healthcare needs of seniors by providing the highest level of care to patients by utilizing the best practices in the field of geriatrics. While the clinical staff at the CFHA provides specialty consultations on many different health issues related to aging, the referrals seen most often are evaluations/assessments for:
- Memory loss (comprehensive evaluation and differential diagnosis to distinguish treatable memory loss from progressive memory disorders)
- Falls and Falls Risk Assessment
- Urinary Incontinence
- Mental Health Assessment and Treatment (depression, anxiety, other mood disorders)
- Driving Assessment (comprehensive evaluation to determine whether a senior is able to continue to drive safely or if driving should be modified or ceased based on findings of physical or cognitive deficits)

Alzheimer's Clinic
Given the tremendous impact Alzheimer's disease can have on patients and their families, the Alzheimer's Assessment Clinic at the Center for Healthy Aging uses a multidisciplinary team approach to assess the patient and develop an on-going support plan for patient and family. This comprehensive program includes a complete medical evaluation, psychosocial assessment, and neuropsychological testing as needed.
Health Screenings

Outpatient Services
To help eliminate time-consuming trips to doctors' offices and the hospital, we have made sure that the prevention, diagnosis and treatment service required by most seniors is available in one location. The clinical staff at the Center for Healthy Aging includes board-certified geriatricians, psychiatrists, internists, clinical nurse specialists and social workers.
Our case management includes help with coordinating services like in-home care, transportation, meal delivery, support groups and referrals to other specialists. Other services available on-site are: Outpatient Mental Health Service, Outpatient Medical Services, Geriatric Case Management, Spiritual Assessment, Care Coordination with Northeast Health System's Assisted Living, Adult Daycare, Respite and Long-term Care Facilities.
Programs for Seniors
At various times throughout the year, programs, lectures, seminars are scheduled -- all topics of interest on daily living and living healthy. Our Forums & Lectures page will list the current seminars. Many are free of charge.
Specialty Consultation
Occasionally, there are patients who suffer from multiple medical, psychiatric and social problems. Our Specialty Consultation Clinic at the Center for Healthy Aging ensures that these individuals receive comprehensive evaluations from a team of specialists who, in consult with the patient's primary care physician, arrange for appropriate services.

The Center for Healthy Aging is located off Route 62 in Danvers, just 1.5 miles from both Route 1 and Route 128. Its pleasant suburban setting is easily reached by patients throughout the North Shore. There is plenty of free parking and easy access for handicapped patients.
